Connect To Support Hampshire website can help you, and your patients, to find information they need to manage their own care and wellbeing and maintain their independence.
You will find comprehensive advice, information on different care options as well as directories of care providers on this Hampshire County Council-led website. There are also hundreds of groups and activities to keep people active and connected to their community.
A free app version of the website is available from GooglePlay or the Apple app store. The app works offline as well, meaning users can access information and search for services even without internet connection.
